The Rec Pass Card is required to gain access to the Pool/Clubhouse?Tennis/Basketball Courts.

This  system will give Management and the Association better “control” to manage the Pool/Clubhouse/Tennis areas by having a record of all persons that walk through the gates/doors/clubhouse plus it gives those that are paying their dues timely use of the Rec areas and those not paying will NOT have access until dues are paid in full.

Each card has its own individual ID number and is controlled from the Clubhouse office computer. 

Rec Passes can be purchased through the Clubhouse office for $25 each card.

Please read and read this procedure and follow the “step by step” directions.  

Owner signatures are required.

On the website (or you can just get a blank copy at the clubhouse office) you will find a “Rec Pass Form” that must be signed by the Owner of Record for each household. (An owner is defined by the name(s) found on the Warranty Deed). If you own your property as a “single” person then you are obviously the person that needs to sign. If you are joint owners only one owner of record needs to sign the Form. 

You will have 4 different options for receiving your Rec Pass Cards. Below is the step by step procedure:

1. First, fill out ALL the personal information on the top half of the Rec Pass Form (attached below).
2. Select which option you prefer (#1 thru #4) then simply check the blank next to your choice. If your choice requires information (option 3 or 4), make sure to fill in the blanks for that entry.
3. Sign, date and print your name legibly.
4. Send the signed Form to Vallery Harrison, LCAM by either:

· Scan and email to: countrycreekstaff@bellsouth.net, OR

· Print & snail mail to: 620 Country Creek Pkwy, Altamonte Springs, FL 32714, OR

· Just show up at the Clubhouse Office with your picture ID

1. If you elect option 3 or 4 the person picking  up the cards will be required to show a source of picture ID so please tell that person to bring their picture ID.
2. Investor owners, either send a copy of your driver’s license, or have your signature notarized so that the staff will know that YOU are indeed the person who signed the Release Form for the cards.
3. THE ABOVE REQUIREMENTS MUST BE MET BEFORE YOU RECEIVE YOUR NEW REC PASS CARDS!
4. REMEMBER!! If either card gets lost, stolen or broken, please notify the Clubhouse office and have the card turned off and you will need to purchase a replacement card for $25.
